Chronic Conditions
A chronic condition is a condition or disease that is persistent and has long lasting effects. It could also be a disease that gets worse with time. A chronic condition is one that isn't communicable or passed from person to person. These conditions are usually the most common, costly, and preventable of all health problems.
Common features of chronic conditions may include:
- Multiple factors leading to onset
- Influenced by heritability, environmental conditions and/or behaviors
- Have a long development period
- May lead to other complications or conditions
- May result in functional impairment or disability
Many of the most common chronic conditions are are closely tied to dietary choices and sedentary lifestyle as well as to the environments, cultures and behaviors that surround individuals. These other factors are known as the social determinants of health.
Common chronic diseases and conditions include heart disease, cancer, stroke, chronic lung disease, alzheimer's disease, diabetes, chronic kidney disease and substance use disorders/addiction.
Prevention
By addressing social determinants and supporting our community members to make healthy choices, the likelihood of getting a chronic disease can be reduced and quality of life improved.
Healthy Behavior Tips:
- Eat a nutrient dense diet
- Get regular physical activity
- Avoid drinking too much alcohol
- Get enough sleep
- Know your family history/risk factors
- Get preventative screenings from your health care provider
- Make healthy choices at school, work and in the community
