Chronic Conditions

A chronic condition is a condition or disease that is persistent and has long lasting effects. It could also be a disease that gets worse with time. A chronic condition is one that isn't communicable or passed from person to person. These conditions are common, costly, and sometimes preventable health problems.

Common features of chronic conditions may include:
  • Multiple factors leading to onset
  • Influenced by heritability, environmental conditions and/or behaviors
  • Have a long development period
  • May lead to other complications or conditions
  • May result in functional impairment or disability


Common chronic diseases and conditions include heart disease, cancer, stroke, chronic lung disease, diabetes, and chronic kidney disease.

Risk factors for common chronic conditions often include components that may be adapted or improved throughout our lives such as nutrition, physical activity, environment, and individual habits or behaviors. Social determinants of health related to education, economic stability, health care, built environment, and social connections can impact the likelihood that we will develop a chronic condition as well as our ability to receive treatment for these conditions.




Prevention

By addressing social determinants and supporting each other in making healthy choices, the likelihood of developing a chronic disease can be reduced and quality of life improved.

  • Eat a nutrient dense diet
  • Get regular physical activity
  • Limit alcohol consumption
  • Get enough sleep
  • Know your family history/risk factors
  • Get preventative screenings from your health care provider
  • Make healthy choices at school, work and in the community
