Reportable Conditions

To report a notifiable condition in Benton or Franklin County to the Benton-Franklin Health District:

  • During Business Hours: Contact by phone at 509.539.0416
  • After Hours: Contact by phone at 509.543.3851
  • By Fax (Confidential): Fax the case report to 509.460.4377

Parent Letters

When a student is exposed to a communicable disease, it is important that staff and families are promptly notified to mitigate risk of further transmission and prevent potential outbreaks.

These letters may be used to inform parents and guardians of potential exposure or infection to a variety of communicable diseases.
